Poland operates a mandatory public health insurance scheme run by the National Health Fund (NFZ), but many residents opt for additional private medical coverage to reduce waiting times and expand their access to specialists. How the NFZ System Works

The NFZ (Narodowy Fundusz Zdrowia) is financed through compulsory health contributions deducted from wages and self-employment income. Key facts:

  • Employees: the health contribution (składka zdrowotna) is 9% of gross salary, deducted automatically by the employer.
  • Self-employed (JDG): contribution depends on the taxation form — 9% of income (progressive scale), 4.9% (flat tax), or a fixed bracket (lump-sum). In 2026 the minimum contribution is approx. 314 PLN/month.
  • Unemployed / not working: you can register as a voluntary NFZ member (dobrowolne ubezpieczenie zdrowotne) for approx. 600–700 PLN/month, or be covered through a family member.

What Does NFZ Cover?

NFZ entitles you to a broad range of services free of charge at point of use, including:

  • Primary care (GP / lekarz pierwszego kontaktu)
  • Specialist consultations (with a GP referral for most)
  • Hospitalisation and surgical procedures
  • Emergency care (SOR) — no referral needed
  • Maternal and child health services
  • Subsidised medications (cheaper with a prescription)

The main drawback is waiting times: specialist queues of 3–18 months are common in 2026 for non-urgent appointments.

Private Health Insurance — Is It Worth It?

Private medical subscriptions (abonament medyczny) offered by employers or purchased individually typically cost 150–400 PLN/month per person. Major providers include Medicover, LuxMed (Luxmed), Enel-Med and Centrum Medyczne Damian.

What private packages usually cover:

  • Unlimited GP consultations (same or next-day appointments)
  • Most specialist consultations without a referral (1–5 day wait)
  • Diagnostics: blood tests, imaging (USG, X-ray, sometimes MRI)
  • Physiotherapy sessions (in higher tiers)

What private subscriptions often do not cover: hospitalisation, major surgery, oncology treatment, dental (separate package), chronic disease medication.

NFZ Registration — Step by Step

  1. If employed: your employer registers you automatically with ZUS and NFZ.
  2. If self-employed: register as a ZUS contributor (DG registration via CEIDG) — NFZ coverage follows automatically.
  3. If moving from abroad: bring proof of deregistration from your home country's insurance system and register at the local NFZ branch (oddział NFZ) with your PESEL number.
  4. Choose your GP within 14 days — declare your primary doctor at the practice (deklaracja wyboru lekarza POZ).

Foreigners and NFZ in 2026

EU/EEA citizens with a valid EHIC (European Health Insurance Card) are entitled to necessary medical care under NFZ during a temporary stay. For long-term residents with a Polish residence permit, the same rules as for Polish citizens apply — you must contribute to NFZ through employment or voluntary insurance.

Ukrainians with a Temporary Protection (UKR) status are covered by NFZ under the special law (ustawa o pomocy obywatelom Ukrainy).
